How to fill my basic details?
Create your clinic/hospital profile
1. Enter Basic Details of hospital/clinic
- Enter the clinic/hospital complete name.
- Select type of medical establishment
- Enter owner name,
- contact number,
- Email id.
- Upload front view image of clinic/hospital building.
2. Details of Address of medical establishment
- Enter full address of clinic/hospital
- Select the city
- Select District
- Select state
- Enter Pin Code
- Upload a scanned copy of address proof (ElectricityBill/Telephone bill/Municipal tax)
3. Details of medical establishment Registration
Enter the following details are-
- a. Government Authority- Enter registration number, year of registration and authority name. Upload a scanned copy of the registration certificate. (Optional)
- b. Association Authority- registration number, year of registration and authority name. Upload a scanned copy of the registration certificate. (Mandatory)
Now save and update.
